ADHD Evidence Briefs

Week of April 1, 2026 · Parent Training: What Works and How Long Does It Last?

This issue features 5 cards on parenting & caregiving. "Can parent training really help?" — If you're a parent of a child with an ADHD diagnosis, you've likely asked this at some point. This week, we look at the evidence for which specific components of Behavioral Parent Training (BPT) are effective, how long those effects last, and what research tells us about parenting stress and children's quality of life. Parent training is not a single cure-all — it works through the interplay of multiple components, and consistent practice matters.
01

🧩 Core Components of Parent Training — What Drives the Effect?

📊 Meta-Analysis (29 RCTs) Grade B JAACAP 2022
Research A synthesis of 29 RCTs found that antecedent manipulation (proactively adjusting the environment before problems arise) and reinforcement techniques (linking praise and rewards to desired behavior) are the key effective components of BPT.
Key takeaway Because this is an exploratory analysis, it is difficult to conclude that any single component alone is sufficient. However, it points to "adjust the situation in advance + praise" as the core direction of parent training.
Action Before a problem behavior occurs, try proactively adjusting the environment (schedules, task load, transition warnings, etc.), and practice providing immediate, specific praise when your child shows a desired behavior.

⏳ Do Parent Training Effects Last?

📊 Meta-Analysis (27 studies) Grade B JCPP Advances 2023
Research A long-term follow-up meta-analysis of 27 studies (31 interventions) found that small-to-moderate, statistically significant effects on ADHD symptoms, parenting efficacy, and parent-child relationships were maintained at an average of 5.3 months after BPT ended.
Key takeaway This suggests the effects of BPT are not just short-term. However, very long-term effects (1+ years) have not yet been sufficiently verified.
Action Even after a program ends, continuing to apply learned techniques (praise, environmental adjustment) consistently — and considering periodic professional check-ins — can help maintain the effects.

💙 Why Do Parents of Children with ADHD Feel More Exhausted?

📊 Meta-Analysis (44 studies) Grade B J Emot Behav Disord 2013
Research A synthesis of 44 studies found that parents of children with ADHD experience significantly higher parenting stress than parents in non-clinical comparison groups. ADHD symptom severity, co-occurring conduct problems, and parental depression were the primary predictors.
Key takeaway Feeling high levels of parenting stress is not a reflection of your failure as a parent. Research evidence shows that the ADHD parenting environment itself demands more energy.
Action You may want to ask a professional about parent support groups or parenting support programs that could be helpful. If your parenting stress feels severe, consider seeking professional counseling.

📋 View All Cards This Week
In addition to the three topics above, this week's cards also cover whether parent training is effective for school-age children with ADHD (Card 4) and the impact of ADHD on children's daily quality of life (Card 5).
Card 1: Core BPT components (Meta-analysis of 29 RCTs)
Card 2: Long-term BPT follow-up effects (Meta-analysis of 27 studies)
Card 3: Parenting stress in ADHD families (Meta-analysis of 44 studies)
Card 4: BPT for school-age children (Systematic review of 20 RCTs)
Card 5: Quality of life in children with ADHD (Meta-analysis of 23 studies)
Viewing all five together — core components, long-term effects, parenting stress, school-age applicability, and quality of life — will give you a broader picture.
